Membership Return of Tithes & Offering Request Form
This form is intended exclusively for active members of Logos Fellowship Church who have faithfully returned tithes and offerings. It provides an opportunity for members facing urgent financial need to request short-term assistance for essentials such as gas, groceries, shelter, or direct bill payments.
To qualify, members must have a consistent record of contributions and submit complete information about their current emergency. Supporting documentation and clear details about the type of help requested are strongly encouraged to process your request quickly.
All requests are prayerfully reviewed by the church administration and processed within 1–3 business days if approved.
Sanctuary Usage Request Form
This form is available to both church members and non-members who wish to request the use of the sanctuary for events such as weddings, homegoing celebrations, meetings, or other approved gatherings. Each request is subject to availability and approval by church leadership.
Members: Those who have tithed faithfully for the past 12 months may request a waiver of the sanctuary rental fee and utility charges. Otherwise, members are responsible only for a $15 per hour utility fee.
Non-members: The standard rental fee is $100 per day, plus $15 per hour for utilities. Full payment is due at the time the form is submitted. If the application is not approved, the amount will be refunded.
By submitting this form, all individuals agree to leave the sanctuary clean, neat, and damage-free, and to comply with all church usage guidelines.
